1. completion of training consistent with Part I and III of the ADA guidelines for teaching the comprehensive control of pain and anxiety of pain and anxiety in dentistry and documented administration of 5 cases of conscious sedation
2. ADA accredited post-doctoral training program
3, completion of 18hr enteral conscious sedation course
4. completion of ADA accredited postgraduate program in pediatric dentistry
5. enteral conscious sedation in the competent manner for 5 years preceding
