A Fund's requirement that the employer prove prior knowledge of the pre-existing disability can be
met by all except which one of the following:
Proof of prior compliance with ADA.
A prior employment physical examination.
An affidavit from the adjuster.
Affidavits from supervisors or managers who know the employee personally may be accepted
evidence.
